If he were in UK and  really a heart patient, he may need to wait longer for any operation or procedure because of smoking.

We are not sure whether his Dr failed to advise him or he is stubborn in failing to stop smoking. Or he think or is sure that smoking is nothing to do with his ‘disease’.
